We reserve the right to refuse treatment to anyone we deem not appropriate for Ketamine Assisted Therapy due to medical or mental health reasons.
Step One- Click For The Ketamine Referral Form
Obtain a referral from your Primary Care Provider or your Psychologist.
Step Two- Click For The Physician Assessment Intake Form
Print off our physician assessment form, and take it to your primary care provider to fill out.
Step Three- Click For The Physician Clearance Letter
Book an intake assessment with a member of our team. Bring filled out intake form with you.
Step Four
Book assessment with our psychiatrist for an assessment. *Please note not all applicants will make it to this step. You must be deemed a good fit for the program before moving forward with treatment.
Step Five
We work with you to determine best course of action and develop a treatment plan.
Step Six
If it is determined that you are a candidate for Ketamine Assisted Therapy, book two prep sessions with one of our psychedelic integration trained psychologists to prepare you for Ketamine Assisted Therapy. *This may require more than two sessions
Step Seven
Book a test dose to make sure you don't have an adverse reaction, and meet with one of our psychologists to discuss treatment goals.
Step Eight
Begin your treatment plan.
